K-12th Grade Enrollment Information

To enroll your student, please visit our enrollment office in person.

Please bring all the following documents  for each enrolling child:

1. Proof of Home Address(Current Utility bill or lease agreement under parent's name and address)

2. Birth Certificate

3. Social Security Card

4. GA Immunization Certificate (3231)

5. GA Eye, Ear, Dental, and Nutrition Certificate(3300)

6. Picture I.D.

For your convenience, enrollment packets are available to print and complete before coming to our office.
